Before completing the form, please review the following requirements to be a Godparent or Sponsor:

I am a member in good standing of Saint James Parish and/ or Saint Katharine Drexel Parish who believes, professes and lives the teachings of Jesus as taught by the Church.
I am a practicing Catholic (attending Mass on Sundays and holy days) who has received all of the Sacraments of Initiation (Baptism, Confirmation, Eucharist)
I am at least 16 years of age.
I am designated by the parents of the child/person for whom I will stand.
If you are married, you must have been married in the Catholic Church, or with the approval of the Catholic Church. Please note that being divorced does not preclude you from acting as a Godparent or Sponsor. However, remarriage outside the Catholic Church places one outside of Communion with the Church and precludes one from acting as a Godparent or Sponsor.
